Any stage of disease: Can begin at diagnosis and continue through treatment to improve quality of life.
Settings: Provided in hospitals, clinics, long-term care facilities, or at home.
Multidisciplinary eligibility: Offered by a care team that works alongside your existing medical providers.
Referral triggers: Ideal for uncontrolled symptoms, repeat hospital visits, or caregiver stress.

Symptom management: Relieves pain, nausea, fatigue, and other symptoms with personalized treatment plans.
Emotional & psychological support: Provides counseling for emotional distress and support for both patients and families.
Spiritual & cultural support: Offers chaplain services and honors each person's faith, culture, and values.
Care coordination & communication: Explains conditions, supports decision-making, and ensures coordinated care.
Support for families & caregivers: Gives education, respite care, and guidance to reduce caregiver burden.
Multidisciplinary team-based care: Care is delivered by a team of medical, emotional, and spiritual professionals.
